Mika Scribe Note Templates are designed to help doctors and clinicians document efficiently and consistently. Each template is pre-made by Mikata with a well-configured AI model (aka jellybeans/suggestions) embedded into the template. These templates can be customized by users to fit their workflow and preferences, see Template Customization for more details.

Template Types

Mika provides four main template types, each tailored for different clinical documentation purposes.

Note

  • Free-form text templates for internal documentation

  • Designed for doctors to record appointment or encounter details. These templates are for internal records and not usually shared outside the clinic.

  • Examples include:

    • SOAP Note

    • Emergency Medicine & Hospitalist Note

    • Clinical Consultation Summary

Letter

  • Free-form text templates for external communication

  • Used to create formal correspondence that can be sent to external recipients, such as specialists or referring physicians.

  • Examples include:

    • Consult Letter

    • Referral Letter

    • Detailed Mental Health Assessment

Form

  • For standardized medical documentation

  • Mika builds pre-made medical forms commonly used in clinical practice and automatically fills in patient and appointment data.

  • Examples include:

    • Lab Requisition Form

    • Patient Enrolment Form

    • Specialist Request Form

Patient

  • For patient-facing communication

  • Designed to create documents or messages that can be shared directly with patients or printed materials to patients.

  • Examples include:

    • Patient Handout

    • Sick Note

  • All templates come with AI-powered jellybeans and suggestions that help auto-populate patient and appointment details.

  • Users can edit, duplicate, and customize templates in the template library.

  • Each template type supports saving, printing, and downloading for easy record management.


About Forms

The Form feature allows doctors to complete structured medical documents quickly and accurately. These forms are implemented using PDF technology, providing the exact layout and structure used by hospitals, clinics, and healthcare organizations โ€” but enhanced with Mika AIโ€™s intelligent suggestions.

Mika AI automatically grabs commonly used forms from different organizations and enriches them by adding smart fields and autofill features. Each field is mapped intelligently, allowing Mika AI to auto-fill patient and appointment details for you.

How it works?

Explore Forms prepared by Mikata

  • Check out the Library! Mika AI curates forms from different hospitals, clinics, and organizations, enhanced with autofill and smart field suggestions.

  • Add a form to your personal library just like any other template to make customization.

Use a Form

  • When you add a form to the appointment, Mika AI automatically fills in patient and appointment information.

  • You can also add default field values in the customized template for faster completion.

Add a signature

  • Add a signature to the form from the toolbar if needed

  • OR Add your signature to the form template so it appears by default each time itโ€™s opened.

    • You can move or replace the signature later within the note.

  • Once completed, you can print or download the finalized form from the toolbar.

  • Once you printed/saved the forms, the finalized form is locked and can't be edited. You can add a new form to edit.

Remarks

  • Form uploads arenโ€™t available just yet โ€” stay tuned!

  • Syncing with EMRs is under exploration, and weโ€™ll share updates once itโ€™s ready.

  • Macros arenโ€™t supported in Forms, but they work in other templates (Note/Letter/Patient).
